Job Description

The Patient Care Coordinator plays an essential role in enhancing the quality of patient services by acting as a liaison between patients, physicians, and other healthcare professionals. This position is ideally suited for individuals with strong communication and organizational skills, as well as a passion for providing exceptional patient care. The Patient Care Coordinator is responsible for ensuring that patients receive the appropriate care in a timely manner and that all necessary administrative and logistical tasks are handled efficiently. This may include scheduling appointments, overseeing patient registration, managing records, and facilitating communication between various departments. Patient Care Coordinators also play a pivotal role in addressing patient concerns and ensuring a smooth healthcare journey from start to finish. Working in various healthcare settings such as hospitals, clinics, and private practices, the Patient Care Coordinator contributes to improved patient satisfaction and contributes to the seamless operation of healthcare services.


Responsibilities

  • Coordinate daily patient schedules and manage appointment bookings effectively.
  • Serve as the main point of contact for patient inquiries and concerns.
  • Collaborate with healthcare providers to ensure comprehensive patient care plans.
  • Maintain detailed and accurate patient records and documentation.
  • Facilitate communication between patients and healthcare professionals to clarify treatment plans.
  • Assist in patient registration and verify insurance information for billing processes.
  • Provide patients and families with information on healthcare services and protocols.
  • Identify and resolve any barriers to patient access to healthcare resources.
  • Communicate with external healthcare providers for referrals and follow-up care coordination.
  • Educate patients and their families about post-care procedures and instructions.
  • Ensure compliance with healthcare laws and regulations, maintaining patient privacy at all times.
  • Gather patient feedback to improve service delivery and healthcare practices.

Requirements

  • Bachelor’s degree in Healthcare Administration, Nursing, or related field preferred.
  • Previous experience in a healthcare setting, preferably patient coordination.
  • Strong organizational skills and the ability to manage multiple tasks simultaneously.
  • Excellent communication and interpersonal skills to interact with diverse populations.
  • Proactive problem-solving skills to address patient needs and concerns.
  • Proficient in using electronic health records and healthcare management software.
  • Knowledge of medical terminology and healthcare practices is highly desirable.
