Job Description

The Patient Care Coordinator is a vital member of the healthcare team responsible for ensuring that patients receive comprehensive and seamless care throughout their healthcare journey. This role requires a perfect blend of administrative skills and empathetic communication, as you will act as a liaison between patients, their families, and healthcare providers. As a Patient Care Coordinator, you will manage patient schedules, coordinate treatment plans, handle inquiries, and provide support to ensure the efficient operation of the healthcare facility. Responsibilities

  • Facilitate communication between patients, families, and healthcare providers effectively.
  • Coordinate and schedule patient appointments and healthcare services accurately.
  • Develop and implement personalized care plans for patients to ensure holistic care.
  • Maintain detailed and accurate patient records in compliance with healthcare regulations.
  • Address and resolve patient inquiries and ensure timely follow-up as needed.
  • Liaise with insurance companies to process claims and verify patient coverage details.
  • Educate patients and families about healthcare processes and available resources.
  • Work collaboratively with multidisciplinary teams to enhance patient care outcomes.
  • Oversee and document the progress and adjustments in patient care plans regularly.
  • Provide compassionate and empathetic support to patients experiencing distress.
  • Manage administrative tasks including data entry, filing, and scheduling duties.
  • Identify and report potential risks or issues in patient care promptly.

Requirements

  • Bachelor’s degree in healthcare administration or a related field preferred.
  • Previous experience in a healthcare setting is highly beneficial for this role.
  • Strong organizational skills with the ability to multitask efficiently under pressure.
  • Excellent communication skills both verbal and written are necessary.
  • Proficient in using healthcare management software and Microsoft Office Suite.
  • Empathetic and patient-centric approach to delivering quality care continuously.
  • Ability to work effectively in a collaborative, multi-disciplinary team environment.
