Job Description

A Patient Care Coordinator plays a vital role in the healthcare system by ensuring that patients receive comprehensive and seamless care throughout their medical journey. This position requires a compassionate individual who excels at communication and has a keen eye for detail. Acting as a liaison between patients, healthcare providers, and insurance companies, the Patient Care Coordinator is pivotal in arranging appointments, clarifying treatment procedures, and ensuring adherence to healthcare plans. They also manage patient records, ensuring compliance with healthcare regulations while maintaining confidentiality. As the focal point of contact, they must continuously demonstrate patience, empathy, and professionalism in all interactions to ensure patient satisfaction and promote the highest standards of patient care.


Responsibilities

  • Coordinate and schedule patient appointments and follow-up visits effectively.
  • Communicate clearly with patients, families, and healthcare providers daily.
  • Maintain and update accurate patient records following medical guidelines.
  • Assist patients with understanding their treatment plans and care procedures.
  • Ensure seamless communication between various healthcare departments and professionals.
  • Handle incoming calls from patients regarding medical queries smoothly and professionally.
  • Verify patient insurance information and ensure paperwork is completed accurately.
  • Provide support for the discharge process to guarantee continuity of care at home.
  • Monitor patient progress and make adjustments to care plans as required.
  • Coordinate with pharmacists to ensure patients receive prescribed medications timely.
  • Keep patients informed about changes in their healthcare schedule or services.
  • Facilitate patient education sessions to bolster understanding of health management.

Requirements

  • Bachelor’s degree in Healthcare Administration, Nursing, or related field preferred.
  • Previous experience in patient care coordination or a similar healthcare role.
  • Strong interpersonal skills with the ability to manage stress effectively.
  • Excellent organizational skills with keen attention to detail and accuracy.
  • Proficiency in medical record-keeping and healthcare management software.
  • Strong understanding of healthcare regulations and HIPAA compliance guidelines.
  • Excellent communication skills, both written and verbal, with diverse groups.
