Job Description

A Patient Care Coordinator plays a pivotal role in the healthcare system, focusing on ensuring that patients receive comprehensive and continuous care during their healthcare journey. This professional is often the first point of contact for patients, assisting them in understanding their treatment plans, scheduling appointments, and coordinating with multiple healthcare providers to ensure a seamless experience. As a key player in patient advocacy, the Patient Care Coordinator works closely with medical staff, insurance companies, and patients’ families to facilitate efficient communication and resolve any issues related to care delivery. Empathy, excellent communication skills, and a thorough understanding of healthcare processes are vital in this role, which contributes significantly to the patients' overall treatment experience and satisfaction.


Responsibilities

  • Coordinate and schedule patient appointments across various healthcare departments.
  • Facilitate communication between patients, families, and healthcare providers effectively.

  • Provide patients with detailed information regarding their treatment and care plans.
  • Assist patients in understanding and navigating the healthcare system efficiently.
  • Manage electronic health records while ensuring privacy and accuracy is maintained.
  • Verify patients' insurance coverage and coordinate authorizations when necessary.
  • Monitor patient progress and follow up on their healthcare appointments eagerly.
  • Resolve patient issues promptly by working with relevant healthcare staff and departments.
  • Develop patient care plans in collaboration with healthcare practitioners regularly.
  • Educate patients about preventative care and health management strategies.
  • Advocate for patient needs regarding quality management and service delivery.

Requirements

  • Associate degree in healthcare administration or a related field required.
  • Minimum of two years experience in patient care coordination or similar roles.
  • Excellent interpersonal skills with the ability to manage multi-discipline communications.
  • Proficient in using electronic health record systems and other healthcare software.
  • Strong organizational skills and attention to detail are highly needed.
  • Understanding of HIPAA regulations and patient privacy laws is mandatory.
  • Exceptional problem-solving skills and a patient-centered approach are essential.
