Job Description

As a Patient Care Coordinator, you will play a crucial role in enhancing the patient experience by ensuring smooth and efficient communication between patients and healthcare professionals. Your primary responsibility is to manage patient care logistics while ensuring high standards are maintained in service delivery. By facilitating communication and coordinating various patient care activities, you ensure that patients receive appropriate, timely, and effective healthcare services. Responsibilities

  • Coordinate patient scheduling and appointments to ensure optimal time management.
  • Act as a liaison between patients, families, and healthcare professionals for effective communication.
  • Maintain accurate patient records, including updating medical histories and treatment plans.
  • Assist patients in understanding their treatment plans, insurance coverage, and billing processes.
  • Organize and oversee patient logistics, including referrals and follow-up care coordination.
  • Ensure compliance with healthcare laws and regulations to maintain patient safety and confidentiality.
  • Proactively address patient inquiries, concerns, and feedback to improve service quality.
  • Collaborate with healthcare providers to implement care plans efficiently and effectively.
  • Manage communication channels to disseminate important information to patients and families.
  • Optimize resource allocation by coordinating available healthcare services and facilities.
  • Monitor patient satisfaction and report on improving patient care processes.
  • Support administrative tasks such as handling medical paperwork and electronic health records.

Requirements

  • Bachelor's degree in healthcare administration, nursing, or a related field preferred.
  • Previous experience in a healthcare setting or patient advocacy role preferred.
  • Strong interpersonal and communication skills to interact effectively with patients and staff.
  • Excellent organizational skills and attention to detail in maintaining patient records.
  • Ability to work under pressure while managing multiple patient care tasks simultaneously.
  • Knowledge of healthcare regulations, insurance policies, and patient privacy laws essential.
  • Proficient in using healthcare information systems and electronic medical records software.
