The Homecare Registered Nurse plays a critical role in the healthcare system by providing vital nursing services to patients in their homes. Responsibilities

• Assess and evaluate patients’ health status and coordinate comprehensive care plans.
• Administer medications and treatments as prescribed by the healthcare providers.
• Monitor patients’ progress and document changes in their health status accurately.
• Manage medical equipment and supplies necessary for patient care in home settings.
• Educate patients and family members on treatment plans and health management.
• Collaborate with physicians, therapists, and other healthcare team members effectively.
• Ensure compliance with all healthcare regulations, standards, and protocols.
• Visit patients in their homes according to scheduled appointments and emergencies.
• Provide post-operative care and wound care management as needed for patients.
• Participate in case reviews, team meetings, and continuous education programs.
• Coordinate with external agencies to facilitate comprehensive patient care.

Requirements

• Valid Registered Nurse license in the state of practice is required.
• Bachelor’s degree in Nursing (BSN) or an Associate degree in Nursing (ADN).
• Minimum of two years of nursing experience, preferably in home healthcare.
• Strong clinical skills, including patient assessment and care planning abilities.
• Excellent communication and interpersonal skills with both patients and teams.
• Ability to work independently and manage a flexible schedule effectively.
• Experience with electronic health record systems and patient management software.
