Job Description

The Patient Care Coordinator plays a critical role in the healthcare delivery system by managing patient interactions, facilitating communication between patients and healthcare providers, and ensuring that patients receive comprehensive care. As the primary point of contact, the Patient Care Coordinator is responsible for coordinating patient schedules, managing healthcare records, and ensuring that all aspects of a patient's care are organized efficiently and delivered smoothly. They work closely with medical professionals to provide a seamless healthcare experience, addressing any concerns patients may have and ensuring their needs are met promptly. This role requires excellent organizational skills, empathy, and a strong understanding of medical procedures and terminology.


Responsibilities

  • Coordinate all aspects of patient care from scheduling appointments to follow-ups.
  • Act as a primary point of contact for patient inquiries and concerns.
  • Facilitate communication between the patient, healthcare providers, and insurance companies.
  • Ensure that patient information is accurately recorded and updated in the system.
  • Assist patients with understanding their treatment plans and healthcare options.
  • Manage patient files and ensure confidentiality of all healthcare records.
  • Work collaboratively with medical staff to streamline patient care processes.
  • Handle administrative duties including billing, coding, and management of patient records.
  • Implement patient care plans as established by healthcare professionals.
  • Monitor patient progress and report any issues to the healthcare team promptly.
  • Ensure that patients attend all scheduled medical appointments and procedures.
  • Provide emotional support to patients and families throughout the treatment process.

Requirements

  • Bachelor's degree in healthcare administration, nursing, or related field required.
  • Minimum of two years of experience in a similar healthcare coordination role.
  • Strong knowledge of medical terminology and healthcare procedures.
  • Exceptional organizational skills and attention to detail are essential.
  • Proficient in using healthcare management software and Microsoft Office Suite.
  • Excellent communication and interpersonal skills to interact with diverse groups.
  • Ability to multitask and handle stressful situations effectively and efficiently.
