Job Description

As a Home Care Nurse, you will play a crucial role in bringing healthcare services to the homes of patients who require medical assistance outside of traditional health facilities. Working in collaboration with physicians and other healthcare professionals, you will be responsible for planning, implementing, and evaluating individualized patient care plans. Your compassionate nature, along with your medical expertise, will help patients and their families navigate the complexities of at-home medical care. Expect to encounter a diverse mix of patients, from the elderly and terminally ill to those recovering from surgery or managing chronic illnesses. Ultimately, your services will enable patients to achieve optimal health outcomes in the comfort of their own homes.


Responsibilities

  • Conduct comprehensive assessments of patient health status and needs in their home environment.
  • Develop personalized care plans based on patients' health assessments and medical guidelines.
  • Administer medications, wound care, and other chronic or acute care interventions.
  • Monitor and document patient's progress and report any significant changes to healthcare team.
  • Educate patients and their families about disease management, medication adherence, and nutritional care.
  • Coordinate with other healthcare professionals to provide integrated care to patients.
  • Ensure compliance with all relevant health and safety standards and practices.
  • Support patients with mobility and daily living activities as needed.
  • Provide emotional support and counseling to patients and their families.
  • Operate health monitoring equipment and perform diagnostic tests when required.
  • Maintain accurate patient records and document care activities diligently.
  • Participate in regular training sessions to stay updated with nursing best practices.

Requirements

  • Registered Nurse (RN) with an active state license to practice nursing.
  • Proven experience in home healthcare or a related nursing field is essential.
  • Strong clinical skills and comprehensive knowledge of healthcare practices.
  • Excellent communication skills to effectively interact with patients and their families.
  • Ability to work independently and manage difficult or challenging patient situations.
  • Empathy, patience, and strong emotional resilience are critical to this role.
  • Strong organizational and time-management skills to manage patient visits effectively.
  • Proficiency in using healthcare technology and electronic health records (EHR) systems.
  • Current CPR certification is required and must be maintained throughout employment.
  • Reliable transportation and willingness to travel between patient homes.
