Job Description

A Patient Care Coordinator plays a vital role in the seamless operation of healthcare facilities, ensuring that patients receive the best possible care throughout their healthcare journey. Serving as a bridge between patients and healthcare providers, Patient Care Coordinators manage the administrative aspects of patient care, from scheduling appointments and coordinating treatment plans to handling patient inquiries and maintaining medical records. They are responsible for fostering a patient-friendly environment where patients feel understood and supported. Often working under pressure, Patient Care Coordinators must have excellent communication skills, a strong sense of empathy, and the ability to strategize and implement efficient workflows. They must be familiar with healthcare regulations and protocols, helping patients navigate the complexities of healthcare systems, insurance, and care options.


Responsibilities

  • Coordinate patient appointments and referrals to ensure timely healthcare delivery.
  • Serve as the primary point of contact for patients' healthcare-related inquiries.
  • Maintain and update accurate patient records and manage medical documentation.
  • Collaborate with healthcare providers to develop comprehensive care plans for patients.

  • Manage insurance verifications and assist patients with billing inquiries.

  • Organize and lead patient care meetings with the healthcare team as needed.
  • Address patient complaints and resolve conflicts to improve patient satisfaction.
  • Participate in the development of departmental procedures and quality improvement initiatives.

Requirements

  • Bachelor's degree in healthcare administration, nursing, or a related field preferred.
  • Proven experience in a healthcare setting, particularly in patient coordination roles.
  • Strong organizational and multitasking skills to handle various administrative tasks.
  • Excellent communication and interpersonal skills to interact with patients effectively.
  • Familiarity with medical terminology and healthcare regulations is essential.
  • Proficient in using electronic medical record systems and healthcare software.
  • Ability to work well under pressure and remain calm in high-stress situations.
  • Empathetic and patient-focused approach to deliver exceptional patient care.
