Job Description

  • Assist in the coordination and management of patient care within a collaborative healthcare team.
  • Support the implementation of care plans and monitor patient progress.

Requirements

  • Educational Qualifications: Bachelors degree in a relevant field such as healthcare, nursing, or social work.
  • Experience Level: 13 years of experience in care management or a related field.
  • Skills and Competencies: Proficient in English language; Strong work ethic; Knowledge of PHRN (Public Health Registered Nurse) practices.
  • Responsibilities and Duties: Ability to work independently and as part of a team in a remote work environment.
  • Qualities and Traits: Strong organizational skills and attention to detail.
  • Working Conditions: Temporary work-from-home position with flexible hours.
