Job Description

A Patient Care Coordinator is an essential role in any healthcare setting, acting as the primary liaison between patients and healthcare providers. The individual in this role ensures that patients receive seamless and effective care by coordinating various aspects of their health and medical services. A Patient Care Coordinator coordinates treatment plans, manages patient schedules, evaluates patient needs, and provides education and support to both patients and their families. This position requires excellent communication skills, empathy, and a strong understanding of healthcare systems and processes. Responsibilities

  • Coordinate patient care activities from admission through discharge to ensure continuity.
  • Communicate effectively with patients, families, and healthcare professionals across all departments.
  • Develop and implement individualized care plans tailored to patients' unique needs.
  • Schedule and facilitate medical appointments and follow-up visits for patients.
  • Assist patients with insurance issues, including authorizations and managing paperwork.
  • Educate patients and families about health conditions, care processes, and medications.
  • Document all patient interactions and care activities accurately and promptly in records.
  • Advocate for patients to ensure they receive high-quality care and service.
  • Monitor patient progress and update care plans based on changes in condition.
  • Address and resolve patient questions and concerns in a compassionate manner.
  • Collaborate with multidisciplinary teams to enhance patient care strategies and outcomes.
  • Ensure compliance with healthcare regulations and protocols during all patient interactions.

Requirements

  • Bachelor’s degree in healthcare administration, nursing, or a related field preferred.
  • Minimum of two years' experience in a healthcare setting, with patient interaction.
  • Strong knowledge of healthcare practices, medical terminology, and care coordination.
  • Excellent organizational and time-management skills to handle multiple tasks efficiently.
  • Outstanding interpersonal and communication skills, both verbal and written.
  • Proficiency with electronic health record systems and basic computer applications.
  • Compassionate and patient-focused approach to care, ensuring patient satisfaction and trust.
