Job Description

A Patient Care Coordinator plays a crucial role in the healthcare system by acting as the main point of contact between patients and healthcare providers. The position involves coordinating patient care activities, ensuring seamless communication, and helping patients navigate the healthcare system. Responsibilities

  • Engage in continuous improvement initiatives to enhance patient care services.
  • Conduct periodic assessments to ensure patient satisfaction and quality of care.

Requirements

  • Proven experience in a healthcare or medical administrative position required.
  • Strong knowledge of healthcare processes and patient care management essential.
  • Excellent communication and interpersonal skills for patient interactions necessary.
  • Familiarity with electronic health records and medical documentation processes.
  • Ability to multitask and manage multiple patient care coordination activities efficiently.
  • Demonstrated problem-solving skills and ability to work in a team-oriented environment.
  • Bachelor's degree in Healthcare Administration, Nursing, or a related field preferred.
