• Monitor and record patient health vitals and communicate any significant changes to the healthcare team.
• Educate patients and family members on treatment plans, medical procedures, and health maintenance tips.
• Coordinate with physicians, therapists, and pharmacists to ensure comprehensive patient care.
• Maintain accurate patient records and ensure confidentiality in compliance with medical standards.
• Develop a rapport with patients, providing emotional support to enhance the patient experience.

Requirements

• Current state nursing license in good standing without any restrictions.
• Minimum of two years of experience in nursing, preferably in home care or a similar setting.
• Strong knowledge of medical protocols regarding home care and patient confidentiality regulations.
• Exceptional interpersonal and communication skills for interacting effectively with patients and families.
• Ability to work independently, managing a flexible schedule with minimal supervision.
• Basic computer proficiency for maintaining electronic medical records and communication.
