As a Case Manager with Interim HealthCare, you schedule the time to see your patients. Youre part of a Home Care Team that works together. Job Responsibilities
Work with one patient at a time.
Arrange your daily and weekly schedule to suit your needs
Develop patients individual care plan
Home visits include medication checks, wound care, IV infusions, and assessments
- Charting completed on iPad provided for you
Good Communication Skills
1 Year Experience
Current RN license/drivers license/CPR
- Participate with a team that supports you
