Job Description

As a Rehabilitation Consultant, you will play a vital role in assisting individuals' return to their optimal level of functioning following an injury or illness. You will work closely with clients, their families, and employers to assess needs, develop tailored rehabilitation programs, and coordinate services to ensure a smooth transition back into work or daily activities. Your expertise in evaluating physical, psychological, and vocational needs will be essential in helping clients achieve their personal and professional goals. You will be part of a multidisciplinary team and must communicate effectively, maintain accurate records, and adapt rehabilitation plans as necessary to accommodate client progress and feedback.


Responsibilities

  • Assess clients' rehabilitation needs through comprehensive evaluations and interviews.
  • Develop individualized rehabilitation plans based on clients' goals and abilities.
  • Coordinate with medical professionals to implement effective rehabilitation strategies.
  • Facilitate communication between clients, families, and healthcare providers.
  • Monitor client progress and adjust rehabilitation plans as necessary.
  • Assist clients in returning to work or daily activities following an injury.
  • Conduct regular follow-up sessions to ensure continued client progress.
  • Provide education and resources to clients and their families about rehabilitation processes.
  • Collaborate with employers to facilitate workplace adjustments for rehabilitation purposes.
  • Maintain accurate and detailed records of client assessments and progress.
  • Advocate for clients' needs within the healthcare system and workplace.
  • Stay up-to-date with best practices and advancements in rehabilitation therapy.

Requirements

  • Bachelor’s degree in Rehabilitation, Occupational Therapy, or a related field.
  • Proven experience in rehabilitation coaching or similar fieldwork is preferred.
  • Strong assessment and case management skills are essential for this role.
  • Excellent communication and interpersonal abilities are necessary.
  • Ability to work collaboratively within a multidisciplinary team environment.
  • Knowledge of workers’ compensation and vocational rehabilitation processes is advantageous.
  • Proficient in maintaining accurate and detailed client records and reports.
