Job Description

A Patient Care Coordinator plays a pivotal role in healthcare settings by ensuring that patients receive comprehensive care and support. This position acts as a bridge between healthcare professionals and patients, coordinating with various departments to deliver seamless care. The Patient Care Coordinator is responsible for managing patient schedules, handling inquiries, and ensuring that patients have access to all necessary services.   • Liaise with insurance companies to verify patient coverage and manage claims processes.
  • Advocate for patient well-being and ensure personalized, compassionate care delivery.
  • Prepare and distribute patient care reports and updates to appropriate parties promptly.
  • Ensure adherence to policies, procedures, and best practices for patient care standards.
  • Engage in continuous professional development to stay current with healthcare trends and practices.

Requirements

  • Bachelor’s degree in healthcare administration, nursing, or a related field is preferred.
  • Previous experience in a patient care or healthcare coordination role is highly advantageous.
  • Strong organizational skills, with the ability to multitask in a fast-paced environment.
  • Exceptional communication skills, both verbal and written, for various audiences.
  • Compassionate demeanor with a patient-first approach to healthcare delivery.
  • Proficiency in medical software, electronic health records, and Microsoft Office Suite.
  • Knowledge of medical terminology and healthcare processes is essential for effective coordination.
