Job Description

The Patient Care Coordinator plays a pivotal role in the healthcare team by ensuring seamless and efficient care for patients across various medical settings. As a key liaison between patients and healthcare providers, the Patient Care Coordinator manages the flow of patient information, schedules appointments, and provides guidance to patients regarding their treatment plans. This role demands a keen understanding of healthcare operations, exceptional communication skills, and a patient-centered approach to service. The Patient Care Coordinator works closely with doctors, nurses, and administrative staff to ensure the highest quality of care is delivered, leading to improved patient satisfaction and outcomes.


Responsibilities

  • Coordinate patient appointments and follow-up schedules across different healthcare departments.
  • Communicate effectively with patients regarding their treatment plans and healthcare needs.
  • Liaise with medical staff to align patient care with the prescribed treatment protocols.
  • Ensure accurate and prompt documentation of patient information in medical records.
  • Act as a point of communication between patients and healthcare providers for any inquiries.
  • Organize multidisciplinary meetings to discuss complex cases and treatment plans.
  • Assist patients in navigating the healthcare system, including referrals and pre-authorization processes.
  • Collaborate with insurance companies to verify patient coverage and clarify billing information.
  • Educate patients and their families about healthcare procedures, medication, and post-care instructions.
  • Implement patient satisfaction surveys and work on strategies for service improvement.
  • Monitor patient progress and adjust care plans in collaboration with healthcare professionals.
  • Ensure compliance with healthcare policies, regulations, and ethical standards.

Requirements

  • Bachelor’s degree in healthcare administration, nursing, or a related field.
  • Minimum of 2 years of experience in a healthcare or patient care setting.
  • Strong organizational skills and the ability to multitask in a fast-paced environment.
  • Exceptional communication skills, both verbal and written, are required.
  • Proficiency in healthcare management systems, including electronic medical records.
  • Understanding of medical terminology and patient care processes is essential.
  • Ability to build and maintain positive relationships with patients and healthcare teams.
  • Knowledge of insurance procedures and patient billing is an advantage.
