Job Description

As a Patient Care Coordinator, you will serve as the central point of communication between patients, healthcare providers, and administrative staff to ensure seamless coordination of healthcare services. You will play a vital role in managing patient care plans, scheduling appointments, and facilitating effective communication among all parties involved. Your primary objective is to enhance patient satisfaction and ensure that each patient's healthcare needs are met promptly and efficiently. With your strong organizational skills and excellent patient service abilities, you will contribute significantly to the healthcare team and help to maintain the quality of patient care. This role requires empathy, attention to detail, and the ability to manage multiple tasks efficiently.


Responsibilities

  • Coordinate all aspects of patient care plans, ensuring they are tailored to individual needs.
  • Manage patient appointments, ensuring appropriate scheduling and minimizing wait times.
  • Facilitate communication between patients, doctors, and other healthcare staff effectively.
  • Maintain and update patient records with accuracy and confidentiality.
  • Ensure that patients understand their treatment plans and healthcare instructions clearly.
  • Respond to patient inquiries and resolve issues in a timely and professional manner.
  • Verify insurance coverage and manage any necessary pre-authorizations as needed.
  • Advocate for patients’ needs and assist in coordinating community or social services referrals.
  • Collaborate with healthcare providers to monitor patient progress and modify plans as necessary.
  • Provide education to patients and families about healthcare processes and preventive care.
  • Track and report on patient metrics to improve service delivery and patient outcomes.
  • Participate in staff meetings and training sessions to stay updated on policies and procedures.

Requirements

  • Bachelor’s degree in healthcare management, nursing, or a related field preferred.
  • Proven experience working as a Patient Care Coordinator or in a similar role.
  • Excellent communication and interpersonal skills to effectively engage with patients.
  • Strong organizational skills with the ability to multitask and prioritize effectively.
  • Familiarity with medical terminology and healthcare procedures is highly desirable.
  • Proficiency in healthcare management software and Microsoft Office Suite.
  • A compassionate and patient-centered approach with a commitment to providing high-quality care.
