We are seeking a qualified and experienced Pharmacist to join our Medical Office of Health (MOH) team at 20J25. As an MOH Pharmacist, you will be a vital part of our healthcare team, responsible for ensuring patients receive the highest quality pharmaceutical care. This role requires the pharmacist to be involved in medication management, patient counseling, and collaborating with other healthcare professionals to optimize patient outcomes. The ideal candidate will possess strong analytical skills, a meticulous attention to detail, and the ability to work effectively in a fast-paced environment. Responsibilities

  • Verify the accuracy and validity of prescriptions and patient medication orders promptly.
  • Collaborate with healthcare professionals to provide optimal pharmaceutical care to patients.
  • Offer expert advice on medication therapy management and patient safety protocols.
  • Ensure proper storage and handling of medications, maintaining compliance with regulations.
  • Conduct medication reviews and support medication reconcilement for patients of all ages.
  • Educate patients and caregivers on medication use, side effects, and interactions.
  • Participate in the development and monitoring of pharmaceutical standards and procedures.
  • Assist in implementing public health initiatives and community health programs.
  • Prepare and dispense medications accurately, ensuring adherence to safety guidelines.
  • Maintain meticulous records of dispensing activities and medication inventories.
  • Report and manage medication errors and adverse drug reactions effectively.
  • Stay updated on the latest pharmaceutical research and industry developments.

Requirements

  • Bachelor's degree in Pharmacy or a related field from an accredited institution.
  • Active pharmacist licensure and certification in good standing with relevant authorities.
  • Minimum of two years of experience in a clinical or hospital pharmacy setting.
  • Thorough knowledge of drug therapy, pharmacology, and pharmaceutical care principles.
  • Excellent communication and interpersonal skills to effectively counsel patients and collaborate with teams.
  • Demonstrated ability to work under pressure and manage multiple tasks concurrently.
  • Proficiency in using pharmacy software systems and electronic health records (EHR).
