Job Description

As a Medical Coder focused on Outpatient (OP) coding with a specialty in handling task code 04J26, you will play a vital role in ensuring that healthcare providers are accurately reimbursed for the services they provide. This position requires a thorough understanding of medical coding services, an ability to interpret complex patient records, and convert them into standardized codes as per ICD, CPT, and HCPCS guidelines. You will work closely with healthcare providers to interpret patient charts, medical histories, and physician reports to convert the necessary information into codes for billing and insurance purposes. This role requires precision, and an analytical mindset to ensure coding compliance and accurate revenue capture for outpatient services.


Responsibilities

  • Accurately translate complex medical records into standardized codes for outpatient services.
  • Review patient charts and medical histories to extract key information for coding.
  • Ensure compliance with prevailing coding standards such as ICD, CPT, and HCPCS.
  • Maintain an updated understanding of coding regulations and guidelines relevant to outpatient services.
  • Collaborate with healthcare providers to obtain additional patient information when required.
  • Audit medical records to ensure accuracy and completeness in coding processes.
  • Enter and update coding information in electronic health record systems with precision.
  • Provide feedback and education to billing staff and healthcare providers on coding practices.
  • Identify and resolve any discrepancies in coding to ensure proper billing.
  • Stay informed of changes in insurance policies that may affect coding practices.
  • Prepare detailed reports on coding activities and present findings to management.
  • Ensure confidentiality and security of patient medical records in compliance with HIPAA.

Requirements

  • Certified Professional Coder (CPC) or equivalent certification is required.
  • Minimum of two years' experience in outpatient medical coding is essential.
  • Thorough understanding of ICD-10, CPT, and HCPCS coding guidelines.
  • Proficient in using electronic health record systems and billing software.
  • Ability to interpret and analyze complex medical documents with precision.
  • Strong attention to detail and excellent organizational skills are imperative.
  • Excellent communication skills to collaborate effectively with healthcare staff.
