Job Responsibilities

• Delivery of safe care and sound clinical decision making
• Assessing and documenting observations of clients condition at each visit
• Initiating/reviewing nursing care plan based on assessment made
• Maintaining accurate nursing documentation
• Performing necessary nursing procedures such as wound dressing, urinary catheter insertion, nasogastric tube insertion, medication packing and etc.
• Conducting regular evaluation of care provided for home care services
• Provide caregiver training and education to client including families on home health care procedures and strategies
• Reporting of any problem incidents encountered in a client’s condition and/or in handling clients and/or caregiver to Nurse Lead
  
  

• Participate in project work to improve the quality of care and services to patients
• Identify service gaps in response to clients’needs and make recommendations to improve service provision of home care
• Participate in integrated Home Health project to provide palliative care services for home health clients which includes telesupport
  
  

• Diploma in Nursing
• 2 years of experience in community nursing/home care
• Proficient in Msoft word, excel, outlook and powerpoint
