• Collaborate with doctors and other healthcare professionals to ensure continuous and cohesive care.
• Provide education and support to patients and their families on health maintenance and treatments.
• Ensure the home environment is safe and conducive to effective medical care.
• Identify patient care requirements by establishing personal rapport with potential and actual patients.
• Maintain accurate and timely patient records, complying with privacy regulations.
• Respond promptly to on-call schedules, addressing any health emergencies.
• Advocate for patient needs and make appropriate referrals to community resources when necessary.
• Engage in continuous professional development to stay current with nursing best practices.

Requirements

• Valid registered nursing license in the state of practice is required.
• Proven experience in patient care, preferably in home care or community health.
• Excellent communication skills to effectively interact with patients and their families.
• Strong assessment and critical thinking abilities for effective patient evaluations.
• High level of empathy and compassion to provide patient-centered care.
• Ability to work independently, manage time effectively, and handle emergencies calmly.
