We are looking for a highly skilled and compassionate Clinical Pharmacist to join our dynamic healthcare team. The ideal candidate will have a strong commitment to providing outstanding pharmaceutical care to patients, optimizing medication therapy, and ensuring safe and effective use of medications. As a Clinical Pharmacist, you will work collaboratively with physicians, nurses, and other healthcare professionals to develop individualized treatment plans and provide education to both patients and staff on medication therapy. The role combines patient care, medication management, and consultative services, requiring in-depth pharmaceutical knowledge and excellent communication skills. Responsibilities

• Review and interpret prescriptions to ensure accuracy and appropriateness of therapy.
• Collaborate with healthcare professionals to optimize medication regimens for patients.
• Provide drug information, education, and counseling to patients and healthcare staff.
• Monitor patient responses to therapy and make necessary recommendations or adjustments.
• Ensure compliance with all regulatory and organizational guidelines regarding medication use.
• Participate in multi-disciplinary rounds and contribute to patient care planning discussions.
• Conduct medication therapy management for chronic disease states and complex cases.
• Identify possible drug interactions, contraindications, and side effects for proper management.
• Develop and implement individualized treatment plans for specific patient needs.
• Participate in quality improvement initiatives and contribute to clinical research projects.
• Maintain accurate and comprehensive documentation of pharmaceutical interventions and services.
• Stay updated with the latest pharmaceutical developments and continuously improve clinical skills.

Requirements

• Doctor of Pharmacy (Pharm.D.) degree from an accredited pharmacy school required.
• Current pharmacist license in the practicing state in good standing mandatory.
• Completion of a pharmacy practice residency or equivalent clinical experience preferred.
• Strong knowledge of pharmacokinetics, pharmacodynamics, and therapeutics essential.
• Excellent communication, interpersonal, and teamwork skills necessary for the role.
• Proficiency in using pharmacy management software and electronic health records required.
• Ability to work independently and make sound clinical decisions under pressure.
