Registered Nurse

A Home Care Nurse provides skilled nursing care to patients in their homes, managing medical treatments, monitoring health, and supporting patient independence while coordinating with healthcare teams.

Key Responsibilities

Home Care Nurses deliver comprehensive in home care tailored to each patient’s needs. Their duties include:

• ·        Patient Assessment and Monitoring: Evaluate and chart patient conditions, monitor vital signs, and assess wound healing or recovery progress. 

• ·        Medication and Treatment Administration: Administer physician-prescribed medications, IV therapy, injections, and other treatments. 
• ·        Wound Care and Personal Assistance: Dress or redress wounds, assist with hygiene,  grooming, bathing, and daily living activities. 
• ·        Care Plan Coordination: Collaborate with physicians, physical and occupational therapists, and other healthcare professionals to develop, implement, and adjust  individualized care plans. 
• ·        Patient and Family Education: Provide guidance on managing health conditions, preventive care, and proper home care techniques. 
• ·        Emotional Support: Offer reassurance and support to patients and families, helping  them cope with illness or recovery. 
• ·        Documentation and Reporting: Maintain accurate records of patient progress, treatments, and communications with the healthcare team. 
• ·        Emergency Response: Be available for oncall emergencies and respond to urgent  patient needs.
