Job Description

A Patient Care Coordinator plays a pivotal role in managing and improving patient care services within healthcare facilities. They act as liaisons between patients, healthcare providers, and various stakeholders, ensuring smooth communication and coordination across various departments. Their primary focus is enhancing patient experiences by streamlining healthcare processes, assisting in the navigation of treatment plans, and ensuring that patient questions and concerns are addressed promptly. Responsibilities

  • Facilitate communication between patients and healthcare providers to enhance service delivery.
  • Coordinate patient appointments, treatments, and follow-up visits for seamless healthcare services.
  • Assist patients in understanding their treatment plans, procedures, and hospital regulations.
  • Act as the primary point of contact for patient complaints and feedback for resolution.
  • Ensure all patient information is accurate, confidential, and updated in healthcare systems.
  • Develop strategies to improve patient care services and enhance patient satisfaction levels.
  • Collaborate with medical teams to develop personalized care plans for optimal outcomes.
  • Identify and address potential barriers to care coordination and patient compliance.
  • Advocate for patients' rights, needs, and preferences during care coordination discussions.
  • Monitor patient progress and work with healthcare teams to adjust care plans as necessary.
  • Educate patients and families about health insurance policies and financial considerations.
  • Organize and lead patient care meetings with multidisciplinary healthcare teams to discuss care strategies.

Requirements

  • Bachelor’s degree in healthcare administration, nursing, or related field required.
  • Minimum of 2 years of experience in a patient care coordination role preferred.
  • Strong knowledge of healthcare regulations, protocols, and patient care practices is essential.
  • Exceptional communication and interpersonal skills required for assisting diverse patient populations.
  • Ability to multitask and manage time effectively in a fast-paced healthcare environment.
  • Proficient in using healthcare management software and electronic health records systems.
  • Empathy and a proactive approach to improve patient care and experience are mandatory.
