Job Description

A Patient Care Coordinator serves as a crucial link between patients and healthcare professionals, ensuring that patients receive the appropriate services and care promptly. They play an essential role in maintaining a seamless flow of information among medical teams, patients, and their families. The Patient Care Coordinator is responsible for scheduling appointments, managing patient records, and educating patients about their treatment options and care plans. Responsibilities

  • Facilitate the exchange of medical information while upholding confidentiality standards.

  • Participate in the development and implementation of patient care improvement plans.
  • Coordinate with insurance companies to ensure coverage and billing accuracy.
  • Support staff training and development in care coordination and communication tasks.

Requirements

  • Bachelor's degree in healthcare management, nursing, or a related field preferred.
  • Proven experience in a healthcare setting, preferably in patient care coordination.
  • Strong communication and interpersonal skills to interact effectively with patients.
  • Excellent organizational skills with the ability to multitask and prioritize tasks.
  • Proficiency with electronic medical records management and healthcare software applications.
  • Empathy and a patient-centered approach to care and customer service.
  • Ability to work collaboratively within a multidisciplinary healthcare team environment.
