We are seeking a dedicated and compassionate Homecare Registered Nurse to join our dynamic healthcare team. As a Homecare Registered Nurse, you will provide essential nursing care to patients in the comfort of their homes. You will be responsible for assessing patient health, creating personalized care plans, and administering medications and treatments to a diverse patient population. The position requires travel to patient homes, so a valid driver’s license and reliable transportation are necessary.

Responsibilities

• Assess patients’ health status and develop individualized care plans for home settings.
• Administer medications and treatments according to physician orders and care protocols.
• Monitor patient conditions and communicate changes to physicians and healthcare teams.
• Educate patients and their families about medical conditions and self-care techniques.
• Maintain accurate and timely documentation of patients’ health records and progress.
• Coordinate with multidisciplinary healthcare teams to deliver cohesive patient care.
• Provide in-home health education to enhance patient knowledge and compliance.
• Evaluate the effectiveness of nursing interventions and adjust care plans as necessary.
• Participate in case conferences and contribute to care decision-making processes.
• Follow up with patients to ensure adherence to treatment plans and scheduled visits.
• Assist with rehabilitative exercises and mobility support to enhance patient independence.
• Respond promptly to patient emergencies and initiate appropriate interventions.

Requirements

• Valid Registered Nurse (RN) license in the state of employment is mandatory.
• Minimum of two years of nursing experience, preferably in homecare settings.
• Exceptional clinical skills with the ability to handle diverse patient needs.
• Proficient in the use of electronic health records and patient care software.
• Excellent communication skills with the ability to educate patients effectively.
• Strong organizational skills with attention to detail and accuracy in documentation.
• Reliable transportation and a valid driver’s license for travel to patient homes.
