Homecare Registered Nurse 27N25

As a Homecare Registered Nurse (27N25), you will play a critical role in delivering high-quality patient care in the comfort of their homes. You will be responsible for assessing patient needs, providing direct patient care, and educating patients and their families about health conditions and ongoing care plans. By adhering to the best practices of nursing, you will ensure safe, effective, and compassionate healthcare tailored to each patient’s unique circumstances. Your expertise will aid in improving patient outcomes, enhancing their quality of life, and ensuring a seamless transition between hospital and home care. This position requires a high level of autonomy, excellent communication skills, and the ability to manage diverse patient caseloads.

Responsibilities

• Conduct comprehensive assessments of the patient’s health and personal needs at home.
• Develop and implement individualized care plans in collaboration with healthcare teams.
• Administer prescribed medications and treatments, and monitor their effectiveness.
• Educate patients and families on disease management, diet, and medication regimens.
• Evaluate and record patient progress and adjust care plans as necessary.
• Coordinate with family members and other caregivers to ensure continuity of care.
• Maintain accurate patient records in compliance with regulations and standards.
• Ensure all medical equipment is operational and used safely during patient care.
• Respond quickly to emergencies and provide appropriate acute care as required.
• Facilitate access to community resources and support for patients and families.
• Adhere to legal, ethical, and professional nursing standards at all times.
• Participate in continuous professional development and nursing training sessions.

Requirements

• Valid Registered Nurse (RN) license .
• Bachelor of Science in Nursing (BSN) degree from an accredited institution.
• Minimum of two years’ experience in homecare, community nursing, or public health.
• Strong clinical skills with experience in administering medications and treatments.
• Proficiency in using electronic health records and maintaining patient documentation.
• Exceptional communication skills to interact effectively with patients and families.
• Current CPR certification and knowledge of emergency response procedures.
