Job Description

As a Home Care Registered Nurse, you will have the unique opportunity to provide nursing care to patients in the comfort of their own homes. Your role will involve working closely with patients and their families to assess health needs, formulate care plans, and deliver compassionate nursing care tailored to each individual’s situation. You’ll be responsible for case management and will coordinate with various healthcare professionals to ensure comprehensive care for the patient. This position requires strong clinical skills, empathy, and the ability to work independently in various home settings. As a female registered nurse, you're expected to uphold the dignity, privacy, and best interests of your patients at all times, ensuring they receive the highest quality care with respect and sensitivity.


Responsibilities

  • Assess patient conditions and develop a customized care plan for home treatment.
  • Administer medications, wound care, and other nursing services as required.
  • Monitor patient health and report any significant changes to the physician.
  • Educate patients and families on health maintenance and disease prevention.
  • Document patient visits and systematically record detailed notes on progress and issues.
  • Coordinate with other healthcare providers to ensure comprehensive patient care.
  • Provide emotional support and education to patients and their families.
  • Maintain patient confidentiality and adhere to ethical standards at all times.
  • Ensure safe home environment, evaluating safety needs and recommending adjustments.
  • Update skills and knowledge regularly through continuing education and training.
  • Respond promptly to patient calls and emergencies to provide urgent care if needed.
  • Manage and maintain an organized record of medical supplies and equipment.

Requirements

  • Valid Registered Nurse (RN) license to practice in the state required.
  • Proven work experience as a Registered Nurse in home care or similar setting.
  • Strong clinical skills with the ability to interact with diverse patient populations.
  • Excellent communication skills and the ability to work independently.
  • Empathetic nature with a high level of patience and attention to detail.
  • Physical stamina to perform tasks such as lifting, bending, and standing.
  • Basic knowledge of home healthcare regulations and best practices.
