A Home Health Care Nurse provides skilled nursing care to patients in their homes, focusing on medical treatment, patient education, and support to help patients maintain their independence and improve their quality of life.

Key Responsibilities

Home Health Care Nurses deliver personalized care to patients in their residences, particularly for individuals recovering from surgery, managing chronic illnesses, or requiring assistance with daily living activities. Their responsibilities include:

• Patient Assessment: Conduct physical assessments, monitor vital signs, evaluate patient conditions, and document medical histories accurately.

• Medication Management: Administer prescribed medications, monitor for side effects, and ensure patients follow treatment plans correctly.

• Wound and IV Care: Provide wound dressing and redressing, manage catheters and feeding tubes, and administer IV therapy at home when required.

• Care Plan Development: Collaborate with physicians, therapists, and family members to create and update individualized patient care plans.

• Patient and Family Education: Educate patients and their families on home care procedures, medication usage, lifestyle modifications, and preventive healthcare practices.

• Emergency Response: Deliver immediate nursing care during medical emergencies and coordinate with emergency medical services when necessary.

• Documentation and Communication: Maintain accurate patient records, document progress, and communicate updates to the healthcare team regularly.

• Supportive Care: Assist patients with personal hygiene, grooming, mobility support, meal preparation, and rehabilitation exercises as needed.
