JOB SUMMARY

The HIM Chart Analysis Specialist is responsible for advanced clerical functions and entails the analysis of medical records for completion upon discharge, by the Medical Staff, and other licensed practitioner who are authorized to document in the medical records in compliance with hospital, regulatory and accrediting guidelines. They understand privacy and security issues related to patient information. The purpose of the position is to perform analysis of all discharges for compliance in accordance with departmental and federal standard.


ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Analyze medical records for completion and assign deficiencies as needed
  • Work involves the analysis and final processing of all incomplete and delinquent medical records distributed or assigned by the HIM staff
  • Work is performed under the supervision of the Health Information Management Supervisor; however, the individual is expected to perform independently and exercise good judgment
  • Analyzes all discharges for compliance with documentation standards, completeness and accuracy in accordance with departmental, hospital, and federal regulatory and accrediting guidelines
  • Records deficiencies on the computer for tracking and reporting purposes
  • Removes deficiencies from the computer upon completion of the medical records
  • Track individual daily productivity
  • Maintain quality score of 98% on a monthly basis

  • Demonstrates proficiency in keyboard operations
  • Privacy and Security: demonstrates an understanding of the importance of providing privacy and security of all patient information
  • Quality Orientation: accomplishing tasks by considering all areas involved, no matter how small; showing concern for all aspects of the job; accurately checking processes and tasks; being watchful over a defined timeframe
  • Knowledge of Health Information Management Principles and Practices and the ability to apply such knowledge to the analysis process
  • Knowledge of TJ, CMS and Medical Staff Bylaws regarding chart completion and documentation standards
  • Knowledge of computer and medical terminology
  • Knowledge of the approved order of assembly for medical records by care unit
  • Knowledge of the importance of timely documentation to prevent delay in the Revenue Cycle
  • Ability to keep open communications with US-based HIM staff
  • Ability to analyze and interpret data relative to incomplete and delinquent medical records
  • Organization: establishes courses of action to ensure that work is completed efficiently; proactively prioritizes assignments and keen ability to multi-task
  • Works Independently: is self-supporting; does not need to rely on others to complete a job


Preferred:

  • At least two years experience in a hospital Health Information Management department
  • Nursing or other Clinical discipline
  • HIM (RHIT/RHIA), Coding or Clinical Credentials Certification is highly desirable but not required
  • Microsoft Office applications and others as required

  • Bachelors degree
