Position Title: Intake Coordinator

Schedule: 9:00 PM to 4:00 AM PH Time

Quick Rundown:
The Central Intake Coordinator will be responsible for managing the referral process from start to finish, working closely with Owners to ensure all National Partner requirements are met. This position involves coordinating all touchpoints (phone, email, portal, Owner, Partner) to deliver a seamless, high-quality experience for all stakeholders.
What You’ll Do:
- Serve as the primary point of contact for Owners and Partners throughout the referral process, ensuring timely and accurate communication.
- Establish strong working relationships with the Central Intake team, Support Center, National Partners, and Owners to facilitate smooth operations.
- Execute all business processes with precision and attention to detail, ensuring consistency and accuracy.
- Manage multiple tasks and prioritize work effectively in a fast-paced environment, maintaining focus on key deliverables.
- Utilize inside sales knowledge to identify and act on opportunities with current partners, supporting growth and development.
- Provide critical support to the Central Intake Director on special projects, reporting, and process improvement initiatives.
- Maintain comprehensive and organized documentation to ensure clear and accurate records.
- Utilize strong verbal and written communication skills to convey information effectively and professionally.
- Apply excellent computer skills, including proficiency in Microsoft Office (Word, Excel, PowerPoint), Microsoft Teams, and specialized databases, to manage daily tasks efficiently.
- Maintain confidentiality of sensitive information and ensure compliance with all organizational and partner policies.
What We’re Looking For:
- Associate degree or equivalent work experience.
- Minimum of 2 years of experience in Case Management, Intake, Support Center, or Contact Center, preferably in Home Care or Home Health.
- Demonstrated ability to plan, organize, and manage projects with a high degree of attention to detail.
- Exceptional follow-up skills, with a proactive, self-starting approach to tasks.
- Strong analytical skills to read, interpret, and act upon referral information and data.
