Job Title: Care Navigator Level 2 (Scheduler)

Work hours: 9:00 AM - 6:00 PM (EDT) (or aligned with timezone as needed)

Work days: Monday to Friday

Salary: $5 - $6 per hour (depending on experience)

Scope of Work/Responsibilities

The Care Navigation Scheduler coordinates in-person medical appointments, manages documentation, and supports the clinical team through accurate data entry and operational tasks. This role requires strong communication skills, attention to detail, and the ability to work comfortably with both patients and provider offices. Experience in medical scheduling is preferred.



Core Competencies

1. Flawless Scheduling & Follow-Through

This is the core of the role.

  • Book in-person appointments accurately
  • Confirm all appointment details with providers
  • Track appointment status and ensure nothing falls through
  • Take full ownership of the appointment lifecycle


2. Accurate Handling of Medical Documentation

Precision is critical in this area.

  • Fax medical records, referrals, and documentation
  • Ensure all documents are sent to the correct recipient on time
  • Support clinicians with accurate and timely data entry


3. Strong, Professional Communication

The role requires regular interaction with:

  • Providers
  • Patients/Members
  • Internal clinical teams


Candidates must be:

  • Clear and confident communicators
  • Professional in tone, especially in healthcare settings
  • Effective at coordinating across multiple stakeholders


Key Responsibilities

  • Schedule in-person appointments with external providers and confirm all details
  • Fax and transmit medical records, referrals, and documentation to provider offices
  • Support clinicians with timely data entry and administrative tasks
  • Maintain and update the Clinically Integrated In-Person Coverage Map
  • Communicate professionally with members, providers, and internal teams
  • Track appointment statuses and ensure follow-up actions are completed

Requirements

Experience requirements

  • Experience in medical scheduling or healthcare administration preferred
  • Strong communication and interpersonal skills
  • High level of organization and attention to detail
  • Comfortable using EHR systems, scheduling tools, and standard office software
  • Ability to collaborate effectively with clinical and operations teams


Basic requirements

  • Must be proficient in speaking and writing English very clearly
  • Must have relevant work experience
  • Be able to submit an NBI clearance and/or Local Police Clearance background check before onboarding [mandatory]
  • Must be available for video meetings with your camera on (when needed)


Technical requirements

  • Device: Reliable laptop or desktop computer.
  • Internet: High-speed connection (minimum 10 Mbps).
  • Audio: Noise-canceling headset.
  • Video: Webcam for virtual meetings.
  • Workspace: Quiet, professional environment.
