Job Highlights

  • Monthly Rate: Approximately PHP 43,000
  • Paid Hours per Week: 40
  • Schedule: Monday to Friday, 9:00 AM – 5:00 PM (on-call availability required for urgent client needs, as scheduled) | Philadelphia, PA, Eastern time
  • Work Arrangement: Work from home
  • Contract: Independent Contractor

Side note: Since this is a permanent work-from-home position and an “Independent Contractor” arrangement, the candidates must have their own computer and internet connection. They will handle their own benefits and taxes. The professional fees are on hourly rates and the rate depends on your performance in the application process.


Responsibilities

Client Management (Virtual):

  • Conduct remote client intake assessments via phone or video and develop personalized care plans in collaboration with families and care staff.
  • Monitor client satisfaction, address evolving care needs, and adjust service plans accordingly.
  • Maintain regular virtual check-ins with clients and families to ensure services are meeting expectations.

Scheduling and Staffing:

  • Use platforms such as HHAeXchange to schedule and coordinate caregiver visits.
  • Respond to schedule changes, call-outs, and emergencies promptly, ensuring visit coverage.
  • Collaborate with the HR department to assign qualified caregivers based on
  • location, availability, and client needs.

Compliance and Documentation:

  • Maintain electronic records and documentation in line with HIPAA and Chapter 611 requirements.
  • Utilize Electronic Visit Verification (EVV) tools to track service delivery.
  • Ensure all documentation, service plans, and updates are current, accurate, and securely stored.

Communication and Collaboration:

  • Serve as the remote point of contact for clients, families, caregivers, and internal staff.
  • Escalate concerns or compliance issues to the appropriate internal departments, including the Compliance Officer.
  • Attend and contribute to internal team meetings via Google Meet, Google Chat, or other platforms.

Quality Assurance:

  • Review and evaluate the quality and effectiveness of remote and in,home care services.
  • Follow up on client feedback, resolve complaints efficiently, and document all outcomes.
  • Participate in service audits and performance reviews to maintain agency standards.


Requirements

  • Bachelor’s degree in healthcare, social work, human services, or related field preferred.
  • Minimum of 2 years of experience in care coordination, home care, or healthcare-related roles.
  • Experience using scheduling and client management systems such as HHAeXchange, Basecamp, or Promise Portal.
  • Strong knowledge of state and federal home care regulations, including Chapter 611.
  • Ability to manage multiple cases and priorities independently in a remote setting.
