Job Description

  • Conduct psychosocial assessments of beneficiaries and their families.
  • Develop and implement individualized care and support plans.
  • Provide counseling, guidance, and emotional support to beneficiaries and caregivers.
  • Coordinate with multidisciplinary teams to ensure holistic care.
  • Conduct home visits and community outreach programs when required.
  • Assist beneficiaries in accessing government schemes, financial assistance, and community resources.
  • Maintain accurate case records, reports, and documentation.
  • Organize awareness programs, support groups, and community engagement activities.
  • Advocate for the rights and well-being of beneficiaries.
  • Participate in team meetings, training programs, and case discussions.
  • Ensure compliance with organizational policies, ethical standards, and confidentiality.
