Job Description

  • Assessment and Intake: Evaluates a residents physical, emotional, and social needs upon arrival to understand their unique background and goals. 
  • Counseling and Support: Offers emotional support to residents and their families to help them cope with grief, stress, life transitions, or illness. 
  • Care Coordination: Collaborates with doctors, nurses, and physical therapists to build personalized treatment and recovery plans. 
  • Discharge Planning: Prepares residents to return home or transition to another facility by arranging necessary equipment, home modifications, and community services. 
  • Resource Connection: Links families to financial aid, legal assistance, transport services, and local community programs
